The Women's Health Nurse Navigator is responsible and accountable for the nursing process in the provision and coordination of patient care. The RN assesses, plans, implements, and evaluates care; coordinates services throughout the continuum of care; provides patient education; promotes self-management; and collaborates with interdisciplinary team members to improve patient outcomes and satisfaction.
- The Women's Health Nurse Navigator provides patient management, care coordination, and disposition planning for Veterans receiving women's health and gender-specific services, including mammography, cervical cancer screening, gynecological care, and bone density testing. The incumbent will coordinate care across VA and community settings and assists Veterans, families, and caregivers in accessing appropriate services to address complex healthcare needs. Major Duties of the Women's Health Nurse Navigator include, but are not limited to: Coordinates and tracks gender-specific care, including breast care/mammography, cervical cancer screening, gynecological procedures, and other women's health services throughout the medical center and Community-Based Outpatient Clinics. Serves as a primary point of contact and resource for Veterans and staff regarding women's health and gender-specific services and collaborates with internal and external healthcare professionals to promote timely access to care. Provides patient management, care coordination, and disposition planning and coordinates care among multiple providers and across VA and community healthcare settings to promote continuity of care. Coordinates community care services and specialty consultations as appropriate, including appointments, required authorizations, follow-up care, receipt of test results, and medical documentation. Tracks the Veteran's progress through the diagnostic and treatment continuum and works to ensure required steps are scheduled, timely, completed, and appropriately followed up. Educates Veterans and caregivers regarding women's health and gender-specific healthcare needs, including the importance of recommended routine screenings and preventive care. Reviews and responds appropriately to mammography reports, BI-RADS, PAP and other gender-specific screening and diagnostic results; enters appropriate results into clinical reminders and ensures reports are available to providers. Collaborates with Women's Health regarding abnormal results. Contacts Veterans regarding critical or missed appointments and follows up with Veterans and/or family members as appropriate to identify and resolve barriers to obtaining recommended mammography, breast cancer screening, cervical screening, and other diagnostic services. Accurately, succinctly, and timely documents patient-care interactions in the electronic medical record using appropriate notes, templates, and clinical reminders. Maintains required tracking systems and spreadsheets, tracks and analyzes clinical data, and accesses metrics and quality indicators in accordance with applicable VHA directives and handbooks. Interfaces with outside referral agencies and supports compliance with applicable regulatory standards while anticipating and implementing preventive patient-care measures. Addresses the psychosocial, nursing, and medical needs of Veterans and their families/caregivers through interdisciplinary collaboration and individualizes care plans and educational interventions based on patient needs. Participates in quality improvement and system redesign activities designed to improve access, quality, efficiency, and patient satisfaction; identifies opportunities for service improvement and provides feedback to management and the healthcare team. Conducts internal Women's Health reviews, retrieves and evaluates applicable data, and supports evaluation of care and outcomes to promote timely and appropriate delivery of sex-specific services. Represents Women's Health on committees and task forces, provides relevant information and data, and actively participates in the Women Veteran Health Committee. Demonstrates leadership in delivering and improving Women's Health care, promotes evidence-based practice, shares knowledge with colleagues, contributes a professional nursing perspective to interdisciplinary discussions, and supports professional development. Performs other duties as assigned.
