This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Health Care for Homeless Veterans (HCHV) Outreach Coordinator provides community outreach throughout the Kansas City VA Medical Center catchment area, access to shelter services, rapid resolution strategies, and housing case management to Veterans who are experiencing or at imminent risk of homelessness. Duties include but are not limited to: Outreach & Engagement Conduct outreach across multi-county VA catchment area, including shelters, encampments, soup kitchens, and community sites. Build partnerships with CoC agencies, SSVF, street outreach teams, hospitals, law enforcement, VSOs, and community groups. Provide "In-reach" through staff meetings, consultative support, and maintaining updated outreach materials. Attend and support community awareness events (Point-in-Time Count, VA2K, Stand Down). Case Management & Housing Support Deliver weekly case management in shelters, streets, and homes for Veterans not enrolled in other housing programs. Utilize Housing First, Harm Reduction, and Trauma-Informed Care models. Assist with housing search, placement, and obtaining furniture/household items. Provide up to 90 days of post-housing stabilization case management. Clinical Assessment & Treatment Complete psychosocial assessments, develop treatment plans, and provide psychotherapy to homeless individuals, families, and groups. Incorporate complex causation in diagnosis and determine service priority for high-need Veterans. Document all care in CPRS and coordinate safe handoffs with providers. Assist Veterans with discharge upgrades, benefits claims, and legal resource referrals. Consults, Triage & Coordinated Entry Manage HCHV Walk-In Clinic and daily National Call Center for Homeless Veterans (NCCHV) consults. Complete Homeless Verification, Rapid Resolution techniques, acuity assessments, and eligibility enrollment for VA care. Enter data into HOMES and HMIS systems and complete referrals to housing, medical, mental health, and benefits services. Program Performance & Compliance Track performance measures, maintain statistical data, and support Joint Commission, CARF, VA, and Coordinated Entry requirements. Develop and evaluate effectiveness of outreach methods and propose system improvements. Additional Requirements Travel throughout catchment area; transport Veterans as needed. Maintain driver's license, complete driving physical, and hold BLS certification. Demonstrate customer service, safety compliance, infection control, and data security.
