LPN Transfer Nurse Coordinator - Transfer Center

The LPN Transfer Nurse Coordinator is responsible for providing professional nursing care relative to room and bed assignment, coordinates room and bed assignments with nursing unites, nurse managers and nursing supervisor; coordinates with physicians and physician’s office staff. Assist Transfer Center Coordinator (TCC) in data gathering. Responsible for admissions in house transfer in STAR.   Education:  Graduate of Licensed Practical Nurse diploma required   Certification/Licensures:  Licensed Practical Nurse state license required, IV Therapy required   Experience:  1 to 3 years experience preferred
