District Health Licensed Practical Nurse(LPN)

Public Health Nurses provide services to individuals, families and population groups. Nursing actions are directed toward the goals of prevention, assessment, risk reduction and health status improvement for individuals, families and communities. Provide supportive or restorative nursing care of clients in  public health settings for clinic, communicable disease, health prevention and health education needs of clients and other duties as indicated. Under direction and supervision of a Registered Nurse.

• Assist with promotion of health for individuals, families and the community through provision of 
• skilled nursing care, provision of personal care activities, provision of health promotion programs, 
• teaching, counseling and appropriate rehabilitation measures.
•  Assist in immunization clinics.
• Maintain clinical records according to established policy and procedures.
• Maintain daily statistics to assist in cost report, division statistics and agency statistics.
• Communicate with client, co-workers and other health professionals to achieve and maintain continuity of patient care in accordance with confidentiality policy.
• Provide care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups. Evaluate outcomes, effectiveness of plan and make changes as necessary.
• Assist with investigation of potential health problems and detection and treatment of diseases or physical/mental abuse problems. 
•  Assume personal responsibility to keep informed of current changes and trends affecting nursing care and professional nurse functions.
• Assist in activities that communicate the range of services available and their benefits. Activities that assist an individual to enroll in a health-related program.
• Participate in activities that require skilled professional knowledge to identify medically at-risk individuals.
• Assist the PHN in evaluating client outcomes to services established and provided in order to determine if goals have been met and to determine need for change in plan of care.
• Provides follow-up contact to ensure that individual receive the services identified in the case plan.
• Perform outreach activities and assessment to facilitate Medicaid eligibility determination and enrollment.
• Assists clients in accessing and using services identified in care plan including advocacy that helps to remove any barriers to services.
• Attend case conferences or team meeting to assess or evaluate individual needs and treatment plans under direction of PHN.
• Assist in response to disaster situations and emergency situations or events in conjunction with agency emergency plan.
• Provide professional health related consultation and assistance to individual providers, agency staff, and the community.
• Participate in activities that support the planning and development of programs. Work with other agencies to improve and/or expand services available to the public.
• Assist in agency evaluation and future goals and objectives.
• Perform other duties as assigned

Must have current licensure as a Licensed Practical Nurse (LPN) in the State of Iowa; Minimum of two years’ experience in general nursing is required. Experience in child health, public health, office/clinic setting and/or home health is encouraged. Mandatory Child & Adult Abuse Reporter Certification and CPR Certification required or willing to obtain; bilingual skills beneficial; computer skills needed; ability to work with diverse populations and to work independently in an unstructured setting with minimal supervision; physical ability to transport essential supplies and to lift up to 50 pounds. Applicants for employment should contact the Human Resources Dept., Room 701 of the Woodbury County Court House between the hours of 8:00 a.m. to 4:30 p.m. for further information.
